U.S. service members have sustained a substantial number of lower extremity injuries from recent military conflicts, leading to amputations or limb preservation procedures. These procedures are often followed by a high occurrence of falls, with considerable adverse effects reported by service members. Scarce research focuses on enhancing balance and preventing falls, particularly within the dynamic population of young, active service members, including those with lower-limb prosthetics or limb loss. In order to fill this lacuna in research, we examined the success of a fall prevention training program for service members who had experienced lower extremity trauma, through (1) quantifying the rate of falls, (2) measuring improvements in trunk control, and (3) assessing skill retention at three and six months following the training.
Participants with lower extremity trauma, including 20 individuals with unilateral transtibial amputations, 6 with unilateral transfemoral amputations, 5 with bilateral transtibial amputations, and 14 with unilateral lower limb procedures, were enrolled. This group totaled 45 participants, with 40 of them being male and an average age of 348 years (standard deviation not specified). A microprocessor-controlled treadmill was employed to generate task-specific postural disturbances mimicking a stumble. A two-week training program was structured around six, thirty-minute sessions. The escalating ability of the participant was directly reflected in the heightened complexity of the task. The training program's effectiveness was assessed through data collection strategies: prior to training (baseline, duplicated), immediately post-training (0 month), and at three and six months after the training period. Using preoperative and postoperative Cone Beam Computed Tomography (CBCT) images, the accuracy of implant placement was determined by recording linear deviations at the implant apex and platform (in millimeters) and angular deviations (in degrees) following image overlay. Postoperative and intraoperative questionnaires tracked patients' self-reported satisfaction, pain levels, and quality of life.
Thirty participants (with 22 implants each) were recruited for each experimental group. Unfortunately, maintaining contact with one patient was not possible.
